Annexure A is otherwise known as Identity certificate which is provided by a Government employee when applying for a passport. The government employee could be from State Government, Central Government, Statutory Bodies or Public Sector Undertakings. When the spouse or children (under 18 yrs.) of a Government employee apply for passport required enclosing Annexure A.

How to make a Passport Affidavit Annexure A?

Annexure A or Identity Certificate is issued by the Administrative/Controlling Authority of the respective department (employer) of the passport applicant.

Below is the step by step process of how to prepare an affidavit.

  1. Make a draft for an affidavit and get printed on the letterhead of the concerned authority.
  2. Paste the applicant’s photo on it.
  3. Make a self-attestation on the photo.
  4. Take a photocopy of the above-said affidavit
  5. The concerned personnel has to fill in the details in the original letterhead and place the official’s signature and seal at the bottom of the page.

Who should provide Annexure A?

The passport applicants who are mandatory to enclose Annexure A is as follow

  1. Employees of State Government
  2. Employees of Central Government
  3. Employees of Statutory bodies
  4. Employees of Public Sector Undertakings (PSUs)
  5. Spouses and Children (below 18 years) of the employees mentioned above can also provide Annexure A.

What are The requirements To Be Filled in Annexure A?

The details to be filled in Annexure A include-

  1. Applicant Name
  2. Name of the employer
  3. Office address where the applicant works
  4. Date since when the applicant is employed in that office
  5. Applicant’s Identity Card number
  6. Reference No. of the application
  7. Date on which the document was issued
  8. Name and designation of the issuing officer
  9. Address and contact number details of the issuing department
