Document required for a Woman applying for change in name due to marriage

• Photocopy of the Husband’s/Wife’s passport, if any
• Marriage Invitation or Attested copy of marriage certificate issued by the Registrar of Marriage
• Old Passport in original
• Old passport self-attested photocopy of first two and last two pages.
• Residence Proof  – Any two of the following can be provided for residence proof. Aadhaar Card / Election Commission Photo ID card/Water Bill /Proof of Gas Connection/Telephone Electricity bill/ Income Tax Assessment Order.
• Educational proof – 10th + 12th + degree certificate ( if completed  )

How to Change your Name in Passport? (After Divorce)

Any married individual who wants to delete their spouse name in the passport after divorce should submit the following documents along with passport application at the time of appointment.

Document Required for Divorcees applying for change of name

• Divorce certificate- a Self-attested copy of divorce certificate or Divorce decree- Divorce decree copy certified by the court.
• Old Passport in original
• Old passport self-attested photocopy of first two and last two pages.
• Residence Proof  – Any two of the following can be provided for residence proof. Aadhaar Card / Election Commission Photo ID card/Water Bill /Proof of Gas Connection/Telephone Electricity bill/ Income Tax Assessment Order.

How to Change your Name in Passport? (After Re-Marriage)

Any women who want to add their spouse name after re-marriage in their passport should provide the below documents along with passport application at the time of appointment.

Document Required for Change of name due to re-marriage

• Divorce decree/death certificate as the case may be the first spouse.
• Second marriage certificate
• Old Passport in original
• Old passport self-attested photocopy of first two and last two pages.
• Self-attested photocopy of the spouse’s Passport
•  Residence Proof  – Any two of the following can be provided for residence proof. Aadhaar Card / Election Commission Photo ID card/Water Bill /Proof of Gas Connection/Telephone Electricity bill/ Income Tax Assessment Order.

How to change your name if you are Employees of Government/PSU/Statutory Body-

Any individual who works under government sector and wants to change their name then the following documents are required along with the passport application at the time of appointment.

• Document Required for Change of name for Govt employee
• Old passport self-attested photocopy of first two and last two pages.
• Proof Of Present Address – Aadhaar Card / Water Bill /  Telephone (landline or postpaid mobile bill)/ Electricity bill/ Income Tax Assessment Order/Election Commission Photo ID card/ Proof of Gas Connection/
• gazette Notification changing a name in applicant’s department
• No Objection Certificate (NOC) (as per Annexure ‘G’)
  OR
•   Prior Intimation Letter (PI) (as per Annexure ‘H’)
•  Fresh ID Certificate in changed name

How to Change your Name in Passport? (All Other Circumstances)

Any individual who wants their name to be changed in their passport for any reasons should provide the following documents along with the passport application center at the time of appointment in the Passport Seva Kendra.

Documents required for name change -Specify Change Category – Major /Minor

• Name Change Annexure & Paper cuttings of two daily newspapers in original
• Proof Of Present Address – Aadhaar Card / Water Bill /  Telephone (landline or postpaid mobile bill)/ Electricity bill/ Income Tax Assessment Order/Election Commission Photo ID card/ Proof of Gas Connection/ Certificate from Employer of reputed companies on letter head ( need any 2)
• Birth Proof – Birth Certificate/Transfer/School leaving/Matriculation Certificate issued by the school/Aadhaar Card/E-Aadhaar/    Election Photo Identity Card (EPIC)/PAN Card /Driving License ( need any 2)
• Educational proof – 10th+ 12th+ degree certificate ( if completed 10th )
• Mariage proof – marriage certificate or marriage invitation ( if married)
