Annexure I- A Declaration affirming the particulars furnished in the application about the minor. 

Annexure I is an affidavit that has to be enclosed when applying for a minor passport by either one of the parents. It is normally enclosed when the parent is separated but not legally divorced or if the child is born out of wedlock.

Who Should Make The Annexure I?

Annexure I can be done by a person who is:

  • A single parent who is not legally separated from his/her spouse but staying away.
  • A single parent who gave birth to a child out of wedlock

Following details have to be filled in the Annexure I

  • Applicant’s name and gender
  • Name of the minor child

Self-declaration statement that the applicant who is the genetic parent of the child takes care and responsibility for the child in all ways
